The paperwork we ask you to complete is not busywork. It gives us the background we need to care for you safely and well. Your health history tells us about conditions, allergies, and medications that can affect dental treatment, while your patient information keeps our records and any insurance claims accurate. The more complete the picture you give us, the better we can tailor your care to you. If a question does not apply to you, it is fine to leave it blank and we will fill in the gaps together when you arrive.
What to have ready
Gathering a few things before you sit down to fill out your forms makes the process quick. Having these on hand means you can complete everything in one pass rather than hunting for details later.
- Your patient and contact information
- Your dental insurance details, if you have coverage
- A current list of medications and supplements
- Notable medical conditions or allergies
- Any concerns or questions you would like to raise
Filling them out before you arrive
When you schedule, we will make your new patient forms available so you have the option to complete them ahead of time. Filling them out at home, at your own pace, tends to be easier than working through them in a waiting room, and it means more of your appointment can be spent on your actual care. If you would rather do the paperwork in person, that is perfectly fine too; just arrive a few minutes early and we will help you through it. Either way, you can request an appointment first and we will send along everything you need.
How your information is protected
The details you share on your forms are personal, and we treat them that way. Your health information is kept confidential and used only to provide and coordinate your care, in keeping with standard patient-privacy practices. We do not hand your information around casually, and if you ever have a question about how your records are used, you are welcome to ask. Being open with us about your health helps us look after you, and you can trust that what you share stays between you and your care team.
Questions before your first visit
If anything on the forms is unclear, or you are not sure which details matter, there is no need to guess. Call the office and a member of our team will walk you through it, or make a note and we will sort it out together when you come in. We would always rather answer a question up front than leave you uncertain, so reach out whenever something needs clarifying.
